| Complete B form below to "Add or Delete" a user. Then send to listed FAX or Email address at bottom of page. New users must Print, Sign and Date at bottom. If form does not automatically fill in the Client name, please select "Option" on right side of Yellow bar and select "Add Host to Priviledged Location". If Yellow bar is not displayed, select Exclamation Point inside Yellow Triangle on Left Panel. Finally, right click on form and select Print. |
